This is the Hack It Day media waiver,  which gives Hack It Day organizers consent to photograph / videotape / audiotape / film / interview individuals & participants. PLEASE MAKE SURE YOUR PARENT/ TEACHER/ GUARDIAN/ SUBSTITUTE IS PRESENT WHEN READING THIS. *

I, {{answer_45055499}} on behalf of {{answer_45057947}} , give Kids In Tech (KIT) and Toronto Youth Cabinet (TYC) (includes the City of Toronto) permission to photograph, videotape, audiotape, film and/or interview either myself, or the person on whose behalf I am giving permission named below (the "Recordings"), and to publish the Recordings in KIT + TYC (includes the City of Toronto) publications/ material, including marketing and promotional materials, and KIT + TYC (includes the City of Toronto) official website, both now and in the future.

The Recordings shall constitute the exclusive property of KIT + TYC (includes the City of Toronto) and may be reproduced by KIT + TYC (includes the City of Toronto) and anyone it has authorized, without compensation or payment to the individual(s) being recorded or any other person. The Proposal submitted prior to the event is copyright property of the students listed by name on the Proposal.

The Prototype produced during the Hack It Day event is considered a co-production between Kids In Tech's team and the students listed on the Proposal. Kids In Tech (KIT) has the authorization to showcase the Prototypes produced from Hack It Day, without compensation or payment to the individual(s) but with the appropriate credits listed. If a sale of the Prototype occurs, 50% of the proceeds will be distributed to the students listed on the proposal and 50% will be distributed to Kids In Tech (KIT).

If I am providing this permission on behalf of someone other than myself, I warrant that I have the authority to do so.

The personal information on this form together with the corresponding photographs, videotapes, audiotapes, films, and/or interviews is collected under the authority of By-Law 879-2010 and the City of Toronto Act, 2006, s.136(c). The information may be reproduced in KIT + TYC (includes the City of Toronto) publications/material, including marketing and promotional materials, and KIT + TYC (includes the City of Toronto) official Web site.
